Transfer To My Way

If you’d like to transfer from your current home care agency to My Way Home Care, here’s a simple guide:

We will confirm our availability in your area. Once you’ve decided, you’ll need to set a start date with us. You may also need to sign a new service agreement outlining your new care plan.

Call My Aged Care on 1800 200 422 or visit their website. Let them know you want to transfer to a new home care provider. They’ll guide you through the process and help with updating your information.

Notify your home care agency that you’re transferring. You’ll need to agree on an end date for their services. Check any exit fees in your agreement, as some agencies may charge a small fee for ending the service.

My Aged Care will work with both agencies to transfer your Home Care Package to the new provider. This transfer usually takes around two weeks, but the timing can vary.

Once your package has been transferred, your new home care provider will start delivering your services based on your care plan.

What’s in a Level 1 Home Care Package?

A Level 1 Home Care Package helps people who need some support. It can include help with everyday tasks like personal care (such as showering or dressing), help around the house, social support, meal preparation, and transport.

What’s in a Level 2 Home Care Package?

A Level 2 Home Care Package is for people needing more support. It includes all the help in a Level 1 package, plus some extra support. This can cover personal care, help around the house, social support, meal prep, transport, and even some basic nursing care.

What’s in a Level 3 Home Care Package?

A Level 3 Home Care Package is for people with medium-level needs. It includes all the support from Levels 1 and 2 but adds even more help. This could mean nursing care, medication management, visits from allied health professionals (like physiotherapists), and extra help with memory or behavior changes.

What’s in a Level 4 Home Care Package?

A Level 4 Home Care Package is the highest level for people with complex or high-level needs. It includes everything from the other packages but with even more support. This may involve nursing care, managing medications, allied health support, personal care, help around the house, social support, meal prep, transport, and support for memory and behaviour changes.
