My Way Disability and Aged Care is a socially responsible community organisation whose vision and mission is to support meaningful, valuable, and contributing roles and responsibilities in the community.

My Way is a not-for-profit organisation which upholds, promotes, and supports the rights and responsibilities of the persons with disability, families and carers.

My Way recognises and treats people with disability, families or carers with deep respect, dignity, and as equal citizens.

My Way recognises, respects, and supports culturally and spiritually diverse needs of people with disability, families, and carers.

My Way Disability and Aged Care provides individualised, flexible, and creative support services to people with disability, and families, or carers to enjoy good and meaningful life in the community of their choice.

How we do?

My Way support team work alongside individual and their families to co-create, co-design and co-produce support plans based on person needs, goals, preferences, aspirations, and dreams. 

My Way support philosophy put person-centred and roles-based planning at the centre of support delivery. The person-centre planning builds on individual talents, strengths, interests, and potential, and it is about what is important to and for the person with disability.

The planning process occurs in a supportive environment to the person with disability and their family and carers. My Way support services promote strength-based and holistic approach which offers hope; choice and control; opportunities and participations to the person with disability, family, and carers.

 My Way support team valued and recognised a person with disability, family, and carers as firsthand experts of the supports. The individual and family expertise in determining their needs and hopes, and flexibility to self-direct the support plans is recognised and supported.

Hameed Mohammed

Meet our Clinical Nurse and Clinical Educator, Hameed. Hameed has postgraduate qualifications in nursing practice and clinical nursing, including a Master of Healthcare Administration, Policy and Clinical Leadership from Murdoch University. Hameed has indepth experience in acute, complex, medical, surgical, mental health and emergency nursing as well as postgraduate research and teaching.

Hameed is currently working in Community, where he is responsible for providing care to people of all ages in their homes and other community settings. Hameed is passionate about providing high-quality person-centred care to his participants, and he is committed to making a difference in the lives of people in the community. He is also responsible for the education and training of support workers.

Prior to nursing, Hameed was member of Academic Board, Research Committee, Global Leaders Committee, Human Research & Ethics Committee as well as elected University Council member at Curtin University. Hameed was also the chair of Postgraduate Committee, Finance and Risk Committee and deputy chair of Guild Council at Curtin Student Guild in 2021. Hameed brings academic, research and clinical education knowledge and experience to My Way.

Lung Sarum is a caring and experienced Home Care professional serving as a Care Coordinator with a solid background in community services and social work. Lung is deeply passionate about helping individuals access the support they need through home care, working closely with participants to understand and maximise their home care packages.

Lung has extensive experience working with people from diverse cultural backgrounds, including CALD (Culturally and Linguistically Diverse) communities. Lung’s empathetic approach fosters trust and ensures that support is tailored to respect each person’s unique needs and circumstances.

Lung excels in connecting participants to appropriate services, finding effective solutions, and advocating for their well-being. Over the years, Lung has provided vital support to individuals and families facing challenges such as homelessness, family violence, and housing instability. By creating personalised care plans, coordinating services, and conducting regular follow-ups, Lung ensures that participants receive consistent and effective assistance.

Collaboration is a key aspect of Lung’s role. Lung works closely with other service providers and organisations to deliver the best possible outcomes for participants. Additionally, Lung has been actively involved in community programs, such as health education sessions and outreach activities, helping people access crucial resources and services.

With qualifications in Community Services, a strong work ethic, and a genuine desire to help others, Lung Sarum is a valuable member of the My Aged Care team at My Way. Lung’s dedication and expertise make a meaningful difference in the lives of those served.

Carmel Matira

Carmel Matira is a Business Operations Manager at My Way – Geraldton branch.

Carmel holds a bachelor’s degree in international hospitality management and her previous role as a registered Property Manager. While being new to this field, she is currently applying her knowledge and experiences in her role to be able to contribute efficiently to the delivery of quality service and care.

She is responsible for the management and leadership of services that support people across their lifecycle and in all domains of life, to enable access to a quality life with dignity in the community, in control of their lives and their supports and services, respect and social inclusion.
