Home Care Support Worker Nomination Form

Do you know someone who would like to be part of your support team at My Way Home Care? They have already done a great job helping you.

This could be;

• a friendly neighbour who helps you out
• a support worker you like
• someone from your church or community group who assists you
• a retired friend whose company you enjoy.

If the person is suitable, we can train and recruit them to be part of your paid support team. This arrangement doesn’t suit everyone, but it can work well sometimes.
