What is Clinical Care in the Support at Home Programme?

If you’re part of the Support at Home programme, you might hear about “clinical care” as one of the key supports available. Clinical care is all about getting professional health help right at home, so you can stay comfortable and independent. It’s tailored just for you, and we’re here to make it feel straightforward and supportive!

What is Clinical Care?

According to the official Support at Home service list from the Department of Health and Aged Care, clinical care includes community-based nursing care to meet clinical care needs. This means hands-on health support from qualified nurses or clinicians, delivered in your home or community setting. It’s designed to assess, treat, and manage your health conditions safely and gently.

It’s part of the broader services under Support at Home, focusing on your medical and personal health needs. You can access it based on your Support Plan and assessment – no one-size-fits-all here.

Examples of Clinical Care Services

Assessing, treating, and monitoring clinical conditions

Nurses check your health regularly and step in early if something changes.

Administration of medications

These services are funded through your quarterly budget. Clinical care incurs zero participant cost, it is fully subsidised to ensure equitable access.
