How Your Care Works

You’ve been assessed using the Integrated Assessment Tool (IAT), which looks at what you need to stay independent (e.g., help with showering, nursing, or meals).

You’ll get a Support Plan with a Notice of Decision. You are already approved under the existing Home Care Packages programme so you will qualify for one of four funding levels. These map to one of four transitional classifications upon automatic transition to Support at Home on November 1.
